It is VITAL that you DO NOT ATTEND our practice if:

• You have tested positive for COVID-19
• If you have been a close contact and have symptoms
• If you live with someone that has tested positive for COVID-19.

If you are affected by these conditions, please telephone our office to discuss alternative arrangements with Dr. Rasiah.
